I had an elcs on tuesday. At my pre-op i was told to eat lots and drink loads throughout the day before. It really is nil by mouth from midnight but i was allowed a few decent sips of water to take pre-op meds and stop my mouth from drying up in the morning. I was late morning and didnt notice the hunger too much.

I think the no nail varnish is so they can monitor your bloodflow (they pinch your nail and can see very quickly how things are) so i would suggest you stick to the guidelines otherwise you may be asked to take it off. Probably get away with just a clear top coat though if you really want.

There is also a clip (pulse oximeter) put over the end of your finger, all the time you are in surgery and intermittently afterwards when you are checked, to measure the amount of oxygen in your blood which finger nail varnish can alter the results of.
Maybe get your toes painted instead.
