Tantrums

My granddaughter has frequent severe tantrums. Some lasting for 4hours. She’s 7 years old, nearly 8. She is one of an identical twin.
her tantrums are violent. She self harms by scratching herself. She punches, kicks, screams like a devil etc. it’s affecting her twin badly and occasionally she’ll join in too.
my daughter is at breaking point. These tantrums are every day. Some without warning. She is literally wild.
when she’s happy she’s such a joy but just lately that’s few and far between.
she says nasty things too, wishing people dead etc.
after the tantrum she will not be sorry and completely blanks you if u mention it to her.
my daughter does t give in to her but she still has the tantrums.
we are waiting for her referral appointment in two weeks but that seems like forever away. We are desperate. Any advice please

This sounds awful. Is everything OK at home? Outbursts like this can be a sign of something in the environemnt. Or have you looked at diet? Processed food can have a big affect on kids.

She rarely eats. She checks dates on everything. She thinks everything is either mouldy or tastes funny. She even accused her mum of putting wine in the kitchen taps!
she’s always eaten a good varied diet. Her sister is obsessed with dates on food too x
