DLA application for DD help

So I am in the process of completing the DLA form for DD.
She has no formal diagnosis and CAHMS suggested a neuropathways assessment but we are waiting for that to happen. She is very anxious and often depressed. There are some sensory issues going on and she has attachment issues with me.

Im worried I don’t have any supporting documentation or external people to confirm what I am saying about DD. I am trying to complete it with as much detail as possible. I’m basically trapped at home with her, she rarely goes to school.

Can anyone give me any reassurance or particular advice in completing the form.

Maybe get citizens advice to help you fill them in? They helped me a few years ago. Hope you get it done x

Answer the questions describing her worst days not her better ones. Be factual. Could school write a supporting letter? Gp or Paediatrician, is there any other service involved at all? Who's doing referral?

Keep a diary of anything that will support your application, I sent 7 days worth in when I documented what help he needed each 24 hours, other than that I only had his autism diagnosis report.

I’ve completed the referral paperwork which was sent to her head of year to give to inclusion department, I’ve never met them.

Not sure how useful HOY would be to be honest, they spend a few minutes with DD a week.

I do have a discharge letter from CAHMS. We do have a paediatrician (unrelated issue) and the last time we did mention the anxiety so it’s on the appointment letter. GP was totally useless.

I’ll start the diary asap. I would like to get this in soon. I’ve had no income since Easter and could do with something (even to pay for petrol for ferrying her to and from school).

I assume I need to print this off to send it in. It can’t be electronic?
