Not specifically for teens but 'feel the fear and do it anyway' is a classic.

Google CCI anxiety there are some good Australian free workbooks you can download.

Try 'Overcoming Anxiety' (any of the 'Overcoming' titles are credible CBT books, there might be other relevant ones).

Obviously teenagers vary a lot but Starving the Anxiety Gremlin is pretty good.

The free downloadable workbooks from Moodjuice are good if Starving the Gremlin is too babyish-looking:
