Forgetting very quickly

Hi all - I am wondering if any of you have encountered the following with your child: he forgets extremely quickly. I tell him the sentence to write and in the middle of it, after 3 words, he forgot what he was supposed to write. Or I say : climb one flight of stairs and wave at me. He says he won't forget "this time", but he often does.
Is it something that points to potentially a condition that is diagnosable and anything I can do to improve it?!

Slow Processing Disorder? Does he take longer than expected to complete homework or other jobs? Unable to make decisions quickly?
