Head rest/side impact position (with pic)

This is my almost three year old in a Maxi Cosi Axissfix - it's in grandparents car as when we bought it I hadn't realised that he would have to forward face so soon in it. The head rest will not come up any higher now and I cannot find anything online to tell me how it should be positioned or when it has been outgrown. Does it look too low to you? His eyes are not yet level with the top of the head rest but he does have quite a long head as you can see!

The thing I would be checking is the harness straps - they must be coming out of the back of the seat level with or slightly ABOVE his shoulders. If they are dipping down behind that's not safe when forward facing and the seat is outgrown (general rule - unless your specific manual says differently - is at or below for rear facing but at or above for forward facing).

Other than that I don't know anything specific about that particular seat.
