Are you getting a good number of wet nappies? Do they seem pretty wet or just a little? What are his bowel movements like...frequency, colour, consistency?

If his levels were of a real concern they would have said something right away, certainly they would readmit him if need be.

His tiredness is likely due to the jaundice...if the weather is nice, daily walks in a bit of sunshine can be helpful.

When you have him seen on Friday, ask if someone can take a look at his latch for you. Hopefully they will have someone qualified to do an assessment of his latch and sucking pattern. You might also ask about having him checked for a UTI as that can sometimes be a reason for the jaundice this long.

For now I would try to keep his feeds quite frequent for a few reasons: it will give him more practice with his latch, he may tolerate smaller feeds more easily with his reflux and tiredness, and it may increase his voiding and bowel movements.

It sounds tough - but you're doing really well. The reflux definitely makes lying down horrible for them - gravity keeps the worst of the acid down while they are upright so it's not too surprising that they prefer being held. Exhausting for you though. My DS seemed to like stepping motion - either going up and down stairs for real or pretending to (as I discovered when staying with my sister whose stairs are so steep I feared I'd kill us by falling down). Also he liked to feed little and often. I assumed this was because the milk was an antacid so helped to settle his tummy. Definitely try to check with a BFC if his latch could be improved if it's still really painful. Alternatively see the videos on Jack Newman's site and see if there's anything you could adjust to improve things. Finally, it may be worth taking cow's milk out of your diet for a while as there seems to be quite a high correlation between casein intolerance and reflux. It can takes a week or so before you notice a difference though some people swear their babies improved within days. Hope things get better soon!
