I have a deep hole on the side of my nipple. It is inflamed and pussy. Agony breastfeeding. DD is 10 months. Went to gp this morning, she said it wasn't a bite but thrush and a crack, prescribed thrush cream for me and gel for dd's mouth. told her it was quite deep for a crack but she wasan't too interested. Anyone out there who might know? Worried about being misdiagnosed and getting worse, it is so painful.

Go back and get a 2nd opinion if you're not happy and it's getting worse. It sounds nasty.

ruty · 20/03/2009 11:08

it will probably have to be the out of hours surgery tomorrow, no more appts today. Bugger.

That might be better though. Not that it's tomorrow but that you'll have somebody completely new examining you. hope you get it sorted

ruty · 20/03/2009 15:17

thanks. I'll try the cream today anyway but never heard of thrush causing a hole.

You need this! Expensive but damn damn worth it. These. Had awful gash in one nipple and someone else on here recommended them. Seem to remember she'd used them after a bite.

Are you sure its not an abscess that has burst? I would be tempted to get it looked at again. Is it red and inflammed round the hole? Do you feel feverish and unwell at all? Round our way we have whats called EMdoc where you can dial out of hours and they arrange for you to go the the local hospital but to see a GP there. I am sure its a 24hr service. If you do start to feel unwell and feverish I would be tempted to go as soon as. Hope it improves soon.

it is swollen and red around the hole. think i will have to go tomorrow, still agonising, possibly more so after application of daktarin.

I had an internal something, it was agony agony agony, they lanced it loads of puss came out and it had a hole, it did get better on its own though.

ruty · 20/03/2009 19:55

having looked up abscesses, i'm pretty sure it is that. is it safe for dd to continue breastfeeding if i have an open abscess? she won't take a bottle. Oh shit.

CarGirl · 20/03/2009 19:57

the feeding advisors told me it was fine as their tummies wouldn't be bothered by any puss they swallowed - yuck!

ruty · 20/03/2009 20:01

thanks - i didn't notice any lump beforehand, just the inflamed hole over the last few days - odd.

Where exactly is it? What shape is it and approx what dimensions?

The point where the nipple joins the flat part of the breast is a classic place to get a fissure (crack). I had one when dd was about 11 months, it was very deep and appeared as if from nowhere. I too thought she must have bitten me. It was also flippin excruciating and took over a week to heal. In the end we deduced (with expert breastfeeding help) that it had been caused by one night of feeding all night with little regard to positioning. I can't remember the exact details, but when a baby's not latched on well, that's a classic place to get damage, and in an older baby teeth can also rub and make it worse. It was incredibly sore. As dd was older, I fed normally from that side by day, but stuck with the other breast only at night until it healed. 3 nights of 'rest' was enough to help it heal well. Keeping a wound moist can help too and remember breastmilk is antibacterial and anti-inflammatory.

Sometimes the little bumps on the areola (which can be very close to the nipple) get infected. They secrete an oily substance and often something can be squeezed out of them that looks a bit like a spot/pus (not advisible to try this!). Is that a possibility?

If there's pus and it's red, then it would be a good idea to get a second opinion on whether it's infected. I'd agree that using the out of hours GP is perfectly justifiable.

I got cracks (and still have the scars) from thrush while feeding so it could be that. Worth considering thrush as it's v painful

ruty · 20/03/2009 20:14

sounds similar Grendle except it is a bit pussy and inflamed.

Definitely get a 2nd opinion if you think it's pussy. Antibiotic creams are a possibility if needed (Dr Jack Newman uses one in his "all purpose nipple ointment").

If you think it could be a wound/crack/bite then do remember that the tissue will heal from the inside out, and that the middle should go a whitish/creamy/yellowish colour as it heals, but not in an oozy pussy way. If you keep it moist then this won't form a dry scab and will heal faster. Eventually pink new tissue emerges from underneath.

Mine was red around it, even though not infected.

Hope it improves soon.

ruty · 20/03/2009 22:19

thanks Grendle. really think the Daktarin is not helping.

btw silverettes look great but as expensive as jewellery!

Definately worth every pennie spent though.. i had a deep, wide open gash which had yellow/green puss coming out of it - complete agony. Shooting pains in the breast like thrush and the lot.

I got the Silverette and within approx 36 hours of constant wear i was completely healed. When removing the cup for feeds i could actually see the would healing before my eyes - they are superb.

ruty · 24/03/2009 00:15

oh wow really? i might try to afford them then - thankyou - so hard to heal this bugger.

expressing as we speak as too painful for dd to feed on that side
