Slow weight gain - new mum. Feel upset :( reflux

Since birth had set back as was just breast - until midwife came out and on day 5 she had lost weight so was advised to put her on formula and BF plan back then. The midwife showed me how little I expressed at that point so felt guilty. When machine was on me.

Baby is now 2 months old and has continued with breast and formula feed. In the last few weeks have had another nightmare of health visitor saying she has slow weight gain. She’s gaining but not as it should. I therefore was told to feed feed from HV which caused more sickness so a doctor and paediatrician got involved. They gave opposite advice

HV plan - Feed every 2 hours 100/120ml (from previously feeding at 60ml) This plan then made baby projectile.
Paediatrican - To feed every 3 hour, 10-15 on breast then formula 60ml.

Current nutramigen and using carobel occasionally but feel that it comes out too slow and takes forever to feed!
After it changed to paediatrician plan a week later baby started wanting fed more and more so upped it to 90ml. She was finishing it and no sick and looking for it every 2 hours (week 6-7)

This week it feels we have went backwards - baby is sleeping longer stretches and HV said not to let her sleep too much because of poor weight gain. Baby now sleeping 3 hours and would maybe sleep more. During feed she is sleeping through most of it or she starts and then falls asleep so roughly back to 60ml. I feel caught on continuing breast as scared she’s gaining little then tiring herself so now sometimes i’m just jumpint straight to bottle but even then baby just wants to feed then sleep!
Her sickness lately hasn’t been best - it’s not projectile but as reflux she still squrims/grunts and can be sick if placed on back
She has no temp

This was sick this morning after breast then 90ml. HV said poke babys feet etc and keep them awake during feed and put to babys mouth..I hate that as feel like i’m forcing but then feel guilty if losing? Baby tends to not open mouth when she is full but what do I do if thats just after a 40ml bottle some feed - I’m unsure if thats considered a lot. Normally I don’t get to see how much when sick as muslin or hard to judge.

weight charts from few weeks ago she is now 7lb13 - 2 month old this week. Should she be on more than 90ml by now?

I'm not sure I understand all your post so correct me if I'm wrong.

Baby is petite anyway and is following her line.

She's having breastfeeds and formula top ups.

Have you been to a proper breastfeeding support group? Not all HVs are trained in breastfeeding.

I'd start there and get your latch etc checked.

As for the top ups, are you getting a sense that the formula is an issue? It might be worth swapping to a full bottle feed once a day rather than top ups. It should be clearer then if sickbis every feed (reflux) or just with formula (could be CMPA)

Is there a reason that DD is on Nutramigen?

Does the Paediatrician suspect Cow's Milk Protein Allergy?

If the Paediatrician does suspect CMPA have you been advised to go completely dairy free as well?

Has the baby been weighed at the same time of day each time, on the same scales and gave the scales been calibrated? You can get wildly varying weights using different scales used by different people.

Has baby been checked properly for Tongue Tie? Have a read of the symptoms here? Unfortunately the HV & Paediatrician are unlikely to be able to spot a TT and you will need a Tongue Tie Practitioner if you suspect it could be this.

You've been given some very conflicting advice and I'm not sure the HV is being very good support here.

Firstly how much you pump has absolutely no indication of how much you are producing.

I would check with a BFC for some up to date information and support. Have you got the Helpline numbers?

The bit about letting her sleep doesn't sound right to me and I'd definitely check that one with a BFC.

When my DD's weight plateaued, I fed every 2 hours in the day and she started gaining more.

If she's sleepy, there are some good tips on feeding sleepy babies here on Kellymom.

I think you do need some urgent BFing support though in RL.

Is there a BFing Support Group near to you or a La Leche League?
