Breastfeeding at night

hi all, I am a first time mom to a 4 month old boy (2 months premature) with severe reflux issues. I am breastfeeding and to get any form of sleep, LO sleeps on his own mattress in between me and dad. The first 2 months of being at home he would not sleep unless held upright by me or dad, we worked towards sleeping on a mattress and with the help of medication were even able to get him to sleep in his own bed for 4 nights, then he became painful again and all progress was lost. We can now at least put him on mattress again, but he sleeps a first stretch of about 4 hours tops, then wakes up every 2 hours. We have not been able to put him down during the day because of reflux issues, so not getting sleep during the day either.

I am now wondering if i am misreading signals at night and that just because he wakes up every 2 hours, he might not need feeding this frequent. (We have had a very rough time with LO, any help or tips on how to get him to "detach " from us a bit more would be amazing too)
