Weaning my toddler - what do I do?

DD is 18 months old, she currently feeds on waking and before bed. When I went back to work at 12 months we dropped the lunchtime feed cold turkey - I tried expressing but I couldn't get the hang of it, so nursery started giving her cow's milk, and she's been fine with it. I'd like to wean her gently over the next couple of months; when she feeds these days she latches on and off a lot, gets distracted easily, and just generally seems like she's not feeding that deeply, although she does sign that she wants milk at these times.

Are these signs that she's ready for me to wean her? If so, how do I go about it? Does it matter which feed I drop first, and should I replace them with follow-on milk or cow's milk?
