My 3mo DS2 keeps coming off breast to suck his fist. After a few sucks he goes back to breast. Few sucks then back to fist.

He's constantly got his fists in his mouth.
Could he be teething already?

He's an easily distractible feeder and this is just making feeds even more hard work..!

My DS did this at exactly the same age. In my case I wasn't producing enough milk (as I found when I tried expressing) and he was actually just hungry. I gradually introduced formula (actually most was formula), and he was then fine and stopped fist sucking.
Not saying this is what's happening to you.

Plenty of women can't express despite baby getting more than sufficient milk so please don't stress if you are one of them, it doesn't mean baby is hungry!

Could be the start of teething, or he could just have noticed them and is practicing this new "game" Smile

I think the opposite may be true as he often comes off the breast coughing and spluttering while my breasts spurt milk all over his face ;)

Yeah it may be a new game and just something else to distract him from the task at hand!

My dd started hand suckling then. It's really common and not a sign of hunger (also get a professional to diagnose this - many women can't express but have plenty of milk!).
The majority of friends babies did something chewy/sucky with hands!

Babies find their hands at around this age and just chew and suck everything, when they can grip it will be toys etc. This is when fist sucking became totally useless as a feeding cue because he sucked his hands all day. No teeth yet at nearly 25 weeks. I think it's a development thing in itself rather than a symptom of something. He loves a good chew and suck.

DS did this from 12 weeks and now at 16 weeks has chosen the first two fingers to suck. Think it's normal.

My four and a half month old finger sucks. Several cute photos look like she's giving the 2 finger salute Shock rude baby! lol!
