so many issues - tongue tie, weight gain, reflux

my ds3 is 10 weeks old. he was 8lb 4oz at birth at 5 days old was 7lb 13oz but at 13 days old was only 7lb 14 oz which brought him down to 25th centile having been born on 75th. he has hovered around the 25th ever since. he is gradually slipping down again although is still gaining some weight. at 5 weeks was 9lb 7.5oz today at 10 weeks he was 11lb 6 oz.
I measured his length at home to be on the 91st, the gp measured his head 2 weeks ago at 75th. the hv is not concerned but the disparity really bothers me. he is mostly bf with 1 bottle of formula at 8pm(ish). he had a tongue tie division at 4 weeks but recently had latch issues again. the mw centre can't see him again to check tt as he is over 6 weeks and I will need a gp referral to get him seen at hospital.
Rationally I think moving on to 100% ff is the best option to get some weight on him but he gets v v refluxy after his 1 bottle - I've made a gp appointment but I am concerned if I go in there with a long list of concerns they will think I am overly anxious. I had pnd with ds1 and ds2 so I can understand them being concerned at my concerns. am I being overly anxious? any comments appreciated.

I don't think you are being overly anxious at all. please be kind to yourself.
is there a private lactation consultant you can see to check tt for you?
we found dd2 had severe tie at 8 weeks so feel your worries.
we went to gp with long list of concerns and she was great... i think taking dh with me helped as then it wasn't just shattered mum worrying ... just a thought...
write it all down for your appt and try to remain calm. you care about your baby... that's all that really matters.
