Dd kicking me in c sec scar whilst feeding lying down...

Hi, I have 21 week old dd2 who is ebf and co sleeps from around 3am onwards (a recent achievement, was recently co sleeping all night but I am working on getting her to go into cot now). The trouble is when rooting for a feed, she kicks out violently and also hits and scratches at my breasts and face (she does the hitting and scratching when feeding awake as well, it's like she had just discovered her arms and hands) but I can cope when she does it awake as I am usually feeding her sitting up and she can't get to my scar.

When feeding her lying down in bed it's just so painful when she kicks my scar, she is getting longer now so her feet just reach that critical tender area which is still sore. I have noticed its starting to sting all the time again (although this could be the nerves beginning to repair?). If I try to sleep with a pillow between our tummies dd can't latch correctly or frequently loses her latch and her thrashing and kicking becomes worse and she becomes more awake (obviously don't want that)!

I have not had this problem with dd1 as I stopped feeding her at 3months so she wasn't as active, also I never fed lying down last time, and I didn't have a Caesarian anyway with dd1!

Any tips for either calming the kicking and thrashing or new sleeping positions greatly appreciated. Sometimes I'm nearly in tears with the pain and also if I am asleep myself I have started to violently twist away from her and DH has had to wake me up in case I hurt dd :(
