5 wo fusses while latching then falls asleep minutes after he's on - help!

DS is 5 weeks and has been BFing pretty well after a wobbly start, but this week he's started fussing like mad when I'm trying to latch him (plus he now tries to 'help' by grabbing my nipple with his hands). However, minutes after he's on, he falls asleep... but if I then take him finally away from the breast, he wakes up and goes bananas... until he's latched back on. And repeat. This doesn't happen every session, seems to be evening/night ones mostly.

He's been exclusively breastfed but has had regular bottles of expressed milk to help with weight gain issues. I tried to decrease the bottles once he'd got his birth weight back but he has perhaps one a day still.

Any advice very welcome as I have no clue what's going on as I'm knackered from late night feeding/crying marathons.
