13 month old being rough - how to teach some manners?

dd is 13 months and is getting a bit cheeky when breastfeeding. Acrobatics, legs in the air, twisting and turning with my nipple still in her mouth. Really lazy latch which results in top teeth leaving indentation in my nipple. Won't unlatch when asked - if I try to unlatch her she clamps down and grazes my nipple. It's actually getting pretty unpleasant but I'm in this for the long haul so won't be stopping any time soon. I loved it when dd was over 2 and could actually be reasoned with about breastfeeding. Any tips on getting her to be a bit more focused? also how to avoid teeth mark and unlatch without getting chomped? I swear she looks at my breast like it's a nice piece of steak and chomps right on without a thought.

sorry didn't explain well, dd3 is 13 months. Dd2 was 2.5 when she self-weaned and breastfeeding from aged 2 onwards was much better as she could be reasoned with.
