4.5 year old toilet accidents

My child was fully trained at 3, last year she had constipation and was scared of using the toilet so she had to go on a course of movicol which resolved the issues. But 6 weeks ago she started having multiple accidents without realising it. Before this she would always announce she needed the loo but I now need to remind her to go, she hasn't gone of her own volition in 2 months.

She's having accidents in school, they're now suggesting pull ups which I'm very reluctant to use as surely that won't resolve it? Her doctor did a urine test last month and there was no infection.

I had a new baby a year ago so is it attention seeking? Or is it physical? We've tried star charts, watch alarms but nothing has helped the issue that she doesn't seem to recognise the signs anymore
