Nearly 4 year old won't wear a top

Hi, im at my wits end. My nearly 4 year old wont wear a top and most of the time not his shoes. Its been about 3 weeks now. He does have eczema and had a flare up so obv clothes might have scratched. The eczema is on its way to being better every morning then he undoes it all by scratching when having a tantrum and not putting his top on. His tops are all cotton and soft. He will wear it in nursery but its usually a battle to get it on. Its getting cold and rainy in Scotland but he doesnt care. Ive withdrawn treats, tried ignoring, tried a favourite character tshirt (worked for one day) and offered him going to the cinema, toys etc but nope. Its a vicious circle as his skin would get better if he didnt get upset and didnt scratch, if he had a tshirt on he wouldnt be scratching his skin! He gets piritan and has all the creams for eczema. Im going back to the dr tomorrow. I sense this is more behavioural and a battle of wills that needs fixed! Its getting embarrassing as he looks neglected with his top off, rashy skin and shoes off! Any advice? He can wear a tshirt happily during his time in nursery but wont elsewhere. Argh! Help?!

What are his favorite cartoon characters? Try buying him some shirts that has a print of his favorite character. Maybe that will do.
