niece's reaction everything and what to tell my sister

i have 15 nieces and nephews and well one of them is well.............not right, i'm worried for my sister, her daughter's reaction to everything is crazy, screaming shouting, no one can tell her what to do ever or the screaming starts, she is 9 and this has been going on for years, she cant be around any cousins because they cant look at her without her starting, she went for help but they said she was grand? none of us can cope its that bad, they haven't seen her around other kids so what was the point, she acts like a spoilt little brat, but worse?

She doesn't seem to have any emotion and is constantly trying to get her younger sister in trouble, even when we have a joke with her or say something innocent to her she starts screaming and having a tantrum like a baby, she will do this anywhere? no concept of anything or anyone..all anyone has to do is look at her and she starts, this isn't normal tantrums? everything i want to say..i cant..reading this ? it doesn't seem so bad but we are living a nightmare with her, she gets everything she wants, but is still like this? we cant take anymore of this but when we say it to my sister? she starts crying and blaming the fact that she works and we mind her kids? but my niece has been like this since she was a baby, we are at out wits end? anyone any advice?

I would be seriously contemplating another GP trip, she sounds like she has ASD.
