DS nearly 4.5 STILL having frequent toilet accidents

I toilet drained DS just before his 3rd birthday - we went cold turkey and he got the hang of it pretty quickly. He's been taking himself to the toilet independently for quite a while now, including washing hands etc.

However, over the past 18 months he's never been reliably dry, and there are periods when he regresses. His pants/trousers are frequently wet (mostly with dribbles), even quite soon after going to the toilet. On a bad day he will go through several changes of clothes - at nursery and at home.

He can be pretty good at taking himself to the toilet unprompted, but at other times he doesn't seem to notice the urge to go. Even when I encourage him to go, he still has accidents in between.

He usually won't tell me when he's wet, and when I notice he will often refuse to co-operate with getting changed, and I end up chasing him round the house...

He is usually ok with poos, but still has occasional accidents. He still wears pull ups at night as he is nowhere near ready for night training (we have tried).

I have put this in behaviour/development as we do have other issues with DS as he can be quite challenging (very hard to get him to co-operate/listen). He is worse at times of stress (we moved house over the summer and he changed nurseries), although he seems quite settled at the moment and is still having accidents...
