Re-register baby name

We gave our baby my surname as a middle name and my husband's surname as the baby's surname. In retrospect I am regretting it and would like to hyphenate our surnames for the baby instead.

Am I right in that we can reregister baby's name until they are 12 months, but it appears as a correction on the long form of the birth certificate?

First name up to a year.
Surname any time but the father must be present. .

Surname anytime, father doesn't need to be present if you're married. We've just changed DDs at 4 years upon our marriage, straightforward to do and get a new certificate

Really easy before 12 months just phone registry for your area and they send a form to you. You dont need to go in even.
