I saw this in a sewing shop years ago, and regret not buying either the pattern or the box there and then.
It was like a small tray or shallow box that folded over to make a smaller box about the size of a pencil case. The sides of the tray/shallow box did not fold down flat, but formed three sides of the closed box. There were two parallel folds across the middle of the tray to form the fourth side. The parts of the tray between the folks and the raised sides formed the top and bottom of the closed box.
It was made with quilted fabric, possibly with buckram or some other stiffener as well.
