I have seen a pattern for something I want to make my dd, it is only showing the pattern in 12-18 months and my dd is 3.11. What is the best way to adjust the pattern? Measure dd and alter it? don't want to mess it up as its a lovely pattern!

Have you chosen a yarn? That pattern is for sport weight yarn. You could try a swatch in DK and see if that gave you the right dimensions. But DK might be too thick for what you want it for. Otherwise, you can just add stitches evenly around the garment. How many stitches would you need to add for your daughter's measurements?
