I've been in my current for 20 years I know the job inside out and I am usually happy doing it. Recently I have had a bit of a crisis of confidence at work which I was hoping to try and push through but today i told my manager I'm feeling useless at work and I feel like I let the team down and that I just feel so exhausted.
Ended up crying in her office and feeling like a complete loser.

I had a couple of episodes like this and started on hrt soon after! Massive help. I don't know if that would be appropriate for your age and situation

The way you feel deserves attention - are you overwhelmed by something specific, is it health/hormones, can you get some support?

If you've done your role for 20 years they know your track record is good.

It might help to write down exactly why you feel 'useless'. Pretend you're doing your own performance assessment and provide the evidence to yourself you are underperforming.
Have you made multiple errors of late? Have you revieved negative feedback? Are you missing deadlines? Have you failed to meet targets you once met? Do you feel you've underperformed on a specific occasion, presentation, meeting, and what made you feel that way?
If you struggle to find evidence, then you'll know its not 'real' and it's your mind deceiving you. You can then manage this with reminding yourself of your successes.
If there is evidence your game has dropped, the next step is to consider why. What's changed? Has your work load increased/changed? Are colleagues pulling their weight? Do you feel well? Are you distracted by personal problems?

You sound overwhelmed, would taking a break from work help?

I think my biggest barrier to how I feel sometimes is my health I have a long term health condition and have had cancer in the past and I think both have a impact on my confidence.
I have also thought it could possibly be perimenopause.

Having low confidence at work is horrible @user879980 if you have been doing the job for 20 years there’s no way you’re useless.
I have worked with a few people who have had cancer and I always find them to be the most hard working. I think that when people have had cancer they know what vulnerability feels like and they find strength in themselves to hide it but remember it’s ok to feel vulnerable your human.
