How is she not wearing the same pair of shoes?

There was a thread on this advert a few days ago, sorry I can’t remember where it was but there was a good few comments on it, I’m sure it was about weigh loss jabs.

I couldn’t see the full photo only the snapshot but clicked on it and realised she is. But look at her hand.
and the characters just feel out of place like they have been cut and pasted onto the background. Doesn’t look real
