Need to know more - is the patient adult or child? Any ongoing health issues? What other symptoms? Who took the obs and why?

Quite blurry. Looks like the pulse is raised but everything else is normal (assuming adult). The BP does drop between the two readings though which might be concerning. As pp said more info is needed, especially any other symptoms.

I am not a doctor or any sort of medical profession but when my heart rate was that high for no apparent reason, my GP sent me to A&E.

Normal BP, sats and temp. High heart rate for an adult - but children naturally have higher heart rates so significance depends on age, background and when taken.

If in doubt go to A&E. You can also call the A&E - which ever one you'd normally go to - and ask them/give them info. Or call 111 if you aren't sure and you don't think it's an emergency. They will tell you if they think you need to go to A&E.

All is fine other than HR which warrants a trip to A&E, assuming adult
