Fed up of flakiness, anyone else?

Asked a manager work for an important document. They knew when the deadline was, it was over a week ago. I gave them the benefit of the doubt and gave them 2 days.
Still didn't send it so I contacted them and they said they'd get it done as soon as they got a chance.
Still nothing a week later. Asked them again and was told you'll get it latest by the end of Wednesday, so sorry for the delay.
Now the end of Wednesday has arrived, he's gone home and still nothing.
He says he has the document ready, so it's a case of hitting send. Id understand more if he were struggling with it in some way but it's literally something in his folder that he needs to send me.

I want to be firmer without sounding rude, but I'm really annoyed at being treated this way. He's shown he doesn't value me or my time, he's had several reminders and just keeps making excuses.

Without sounding sexist, I'm wondering if I were male and higher up than him whether he'd still have this can't be arsed approach, I don't think so.

Send another email clearly stating that the deadline has now passed and you haven't received the document despite multiple reminders, could they please kindly send it to you by the end of business day Thursday. Copy in whoever is higher up in a chain that they report to.
You want to play silly games, you get silly prizes. Hopefully, their superior will take them down a peg.

Thank you. He probably thinks I'm a nag or whatever but he's taking me for a fool. I haven't worked here for long and this makes me wonder what else he's going to do or not do.
