Or just nothing? Dd has an image of her toes breaking and a lot of blood. Or this just random

It sounds like an intrusive thought. It doesn't necessarily mean either i dont think. If it's distressing her, definitely worth speaking to the gp.

Based on a few words no one can diagnose her online. Even if they are a doctor. As pp said consider consulting GP or if you feel it’s more urgent she can self refer to mental health (if in England and Wales 111 option 2)

That sounds like an intrusive thought. Lots of reasons it could be happening. OCD, anxiety amongst a range of others

My DD (13) has had thoughts like this for a couple of years, they seem to happen when she's anxious or worried about something. No idea if it's the right thing to do but she finds it helpful to focus on the thought and then change the 'picture' to something nicer. E.g. she has a horrible thought about maggots crawling out of rotting fruit, so she'll make herself imagine putting the rotten fruit into the bin and then cutting up a fresh apple. She has other ones too, really used to scare her when she started having them!

Depends - is this thought affecting her life? Will she not walk or do certain things because she’s worried about her toes breaking?
