Hi I can possibly help.. I am a rmn! What kind of assessment is it do you know? A mental health act assessment or an initial assessment from a secondary mental health team?

Is it just mental health nurses who do the assessments? Is everything discussed confidential or does it end up on your medical record? Thank you.

I’m a clinical psychologist. If it’s a generic mental health assessment I would be asking about family background, significant life events, any experiences of trauma, current symptoms and impact on their functioning, drug/alcohol use, any self harm or suicidal ideation.

Stuff is confidential in that it wouldn’t be shared with family etc but yes I would write notes, share with my team if appropriate, and write a report to GP. Confidentiality should be explained. Mental health assessments could be completed by a nurse, psychologist, social worker or psychiatrist. It depends on the team/service you are referred to.
