How would you spend your time? Would you volunteer with something? Or do your current job? Or something completely different?

I have so much opportunity, but am completely overwhelmed with no confidence.

Take your time and look around. In the US they seem a lot more relaxed about changing jobs.

When did you arrive? I've found with ask my moved I get a honeymoon period of 3-6 months, then it's shit and lonely for 6 months, then you settle. Leave some of the exploring so you've still got things planned for during the shit 6 months

I'm well settled into life here in general, but not been working because of massive family illness.

I like it here very much but need something to do. Feels like I'm wasting time by not building a career.

Well, the blunt answer is start applying for jobs! What else do you want us to say? Presumably you have foreign qualifications and have been out off work for a while, so you might have to look beyond your normal range, which fixes your first question asking if you should look for a career change
