To ask GP to extend medical certificate without appointment?

I’m on medication for my ear at the moment. GP said keep an eye for side effects. I needed two days off work due to side effects from the medication nausea and headache etc. Still not feeling great and feel I need a bit more time to recover and I am exhausted too. Cert was for yesterday and today. AIBU to email GP reception again and ask if doctor will attend cert? Or do I make a new appointment. I’m nervous that he will say no as he’s already given me two days due to the side effects. I also don’t want to see like I am trying to avoid another appointment fee because I’m not. GP surgeries are so busy so I don’t want to go over again unnecessarily. Would you email reception again to extend the cert by two days? And what would you say? Also I’m in Ireland so we can’t self -certify immediately after having the initial certified leave. Can’t think straight atm so please be kind 🙈

I don’t see why you can’t try and if they say no then you could ask for an appointment.
