On an elderly relative. I’m baffled, GP is baffled.

Various things it could be but no positive identification.

The red spots (poss petechiae?) don’t discolour under glass.

Looks like statis dermatitis/ vericose eczema . Occurred on ankles and legs due to poor circulation , occurs in older aged adults. Can turn to ulcers but these look more on the healing side.

Thanks very much for your response, very helpful. That’s one of my top 2 options, I expected the GP to be able to identify it one way or another.

yes a second opinion from another doctor would be good as those two conditions are actually very common in older aged adults so strange the doctor wouldn’t know.

Also used on older aged patients is cream called zerobase zeroderma emollient cream . Works well :)
