Fucking dirty underwear

This isn’t a troll post (although I know that’s exactly what a troll would say) I’ve namechanged because it’s not a topic I want linked to my name but MNHQ can check and confirm that I’m a regular poster and have posted about this problem before (it’s actually changed since my last thread and I can’t actually find it so just starting a new one)

I always get stains in my underwear. Its usually poo, but sometimes it’s blood as well (deep red- not bright red but also not brown like old blood, somewhere in the middle). When it’s poo, I don’t feel it happening and I don’t realise it’s happened until I see it in my underwear, but when it’s blood I can tell because it’s so painful in the entire area and it hurts to walk, sit down etc.

I don’t understand because I ensure I’m completely clean after going to the toilet. Ive checked using a mirror and I’m spotlessly clean, but yet my underwear still end up with significant staining. Ive tried using every type of toilet paper I could get in the supermarket, ive tried wipes specifically for after the toilet, ive tried using toilet paper with warm water, I’ve tried different types of underwear. I’ve tried bathing and showering and using soap. Nothing makes a difference, I still get staining.

I don’t know if it’s leaking or what’s going on but it’s ruining my life. Its sometimes bad enough that it goes through my clothes and I’ve had to leave work or make excuses to get out of social situations because of it. It’s ruining my life- I’m 22 and I’m terrified of doing normal things like going out with my friends and going to work because of it happening. I can’t wear certain clothes I’m case it happens.

I’ve got IBS-D and have frequent attacks with that, but like I said I ensure I’m spotlessly clean and I’ve checked myself using a mirror. My underwear still ends up with marks on it. I have got medical advice before and I don’t have any piles or any fissures and there’s no obvious cause for the leaking or the blood. I’m waiting to see a consultant but it could take a while.

Does anyone have any idea what it could be? I’m not asking for personal stories because that’s innappropriate and I don’t want to be accused of a troll, but I’m posting out of frustration and exhaustion with it all and what I’m wondering is does anyone know what this could be or have any idea how I can stop it happening

I want my life back before this started😥 I’m so frustrated and fed up of it all.

If you havent already, id go back to the GP and ask about a referral to a colorectal team with a view to a colonoscopy or a flexible sigmoidoscopy to check there is no inflammation internally/internal piles. How is your pelvic floor? There may be a way for the GP to refer you to a pelvic floor team/physio for assessment too.?

I don’t understand how anything is getting onto your underwear without you being able to FEEL it happening. Don’t you have butt cheeks?! How is your underwear pressed right up against your anus Confused

Thanks for the advice! I’ve been using liners anytime I know it’s likely to happen, like if it’s happened for the previous couple of days.

I’ve been to the GP and definitely don’t have fissures or piles but I know some haemorrhoids are further up so I’ll need to wait for the hospital appointment to find out if that’s causing it or something else.

It’s actually a relief that other people have had it with IBS too, I honestly thought I was alone with this problem! I’ve cried myself to sleep every night for the last 2 weeks because I felt so horrible about it and so alone.

I’ve never had any problems at my blood test and my calprotectin test was normal which is a big relief, and I can cope with typical IBS symptoms, but this particular symptom is awful.

I’ve tried exercise, fodmap and cutting out food types but nothing has helped it Sad

Yeah of course I have butt cheeks, honestly what was the need for that post? I hope you feel a lot better for posting it.

For what it’s worth, I think the not-feeling-it-happening is possibly part of whatever the problem is. I don’t understand it either hence why I’m waiting to see a consultant.

I think the not feeling that it's happening merits investigation quicker than the routine referral appointment you're waiting for.

It could be caused by all sorts of things like proctitis/IBD, nerve damage, growths etc. all of which would be better treated sooner rather than later.

I have IBD. Although I get a feeling of needing the loo urgently, if I produce blood on its own I don't feel it happening and only know once I feel it has come out.

You have my sympathy OP. It's not a nice thing to have to deal with.

How do you know you havent got fissures or piles? Have you had tests?

You poor, poor thing. My daughter (who’s only 5) gets leaks and it’s utterly miserable.

Hers was caused by constipation, which turned into an impaction. What happens is that a blockage of old poo forms, and new poo leaks around it and out. You don’t feel it. Is there any chance that could be happening to you?

Did you say you’re waiting for a gastro referral? How long away do you think that is? Is there any chance at all you could find the money to go private?

DH bleeds and doesn't know when it happens so of course OP wouldn't be necessarily aware if when it happens to her.

Have you tried eliminating various foods from your diet to see if that changes anything? I have a friend with IBS, and dairy is a massive irritant for her.

You should keep going to the doctors about this, as it's not normal, especially for someone so young.

In the meantime, using a panty liner you can change whenever you notice some soiling, might make you feel a bit more secure.

I am sometimes prone to leakage (not IBS). YUK WARNING - I find that sometimes a soft poo will hang around in my rectum - I clean it out with a wet finger and this generally stops leakage (sorry for the TMI)

When my husband had piles he had some leakage. The consultant suggested he shove a cotton wool pad up against his anus to catch leaks. That worked for him - but he didn't have much leakage. That said, most leaks look worse than they are once they reach the underwear.

Hi be kind to yourself- this is quite common and the more we talk about it the more we find support and solutions - period pants might help as leakages are minimal and allows you to relax a bit when out - keep a food diary as might find it’s juice or simply Duffy water for example - the blood bit worries me too maybe take a sample to gp if you travel it can be picked up in streams and two friends had similar symptoms bloating is the give away for ibs but I have no other advice sorry

You have my sympathy...I get this but because I have piles and it's really hard to get totally clean without using the shower. My husband gets leakage because he had a big piles removal operation which damaged his nerves down below.
