GP's medical form - who pays, me or employer?

I am due to start a job next month and have been asked to give a medical questionnaire to my GP surgery for a GP to complete and sign prior to a contract being issued. I cannot start this role until this form is returned. The charge for this is £40.00. Who pays for this usually? Is it me or is it the future employer who have asked for the completion of this form?

OK, thank you. I wasn't sure as the firm are happy to reimburse travelling expenses for training but not this so that confused me somewhat.

It should be the employer as it is part of the onboarding cost of recruiting a new employee.
You can ask how you reclaim the fee.
E.g. if it should be submitted as an expense claim or if HR just need the receipt etc.
If there is an objection or a refusal to pay it will give you a good indication of the type of organisation you are joining.

I would have thought your prospective employer as they have requested the note.

I had some sick leave in a previous job and they wanted a report from my GP on how my disability affects me. The GP set a price and they paid it.
