Child left unattended at nursery

Posting for traffic. Dc1 is 3.5 doing well and is happy at nursery. I recently looked at the care inspectorate report that details an incident where a child was left unattended. Wtf?
Dc1 really loves the nursery but I’m now unsure about sending dc2 next year when I go back to work.
AIBU ..? I am horrified at this but ive since heard of this happening at another nursery.. are these incidents more common than I think ?

One near me lost their registration for a while as they managed this twice in a single month!

One of the incidents was in the park, one was in a playground setting locked outside with potential for strangers to access. Not a case of particularly high standards…

It doesent give the details, the report just said a child was left unsupervised for a period of time..
rugsofhonour you are absolutely right I do leave them to do things like go toilet, maybe I’m being a bit over the top, I’ll need to speak to the nursery to get the full details I suppose. I shall post if find out
