HELP!!! Passenger locator form struggles!

I know this isn’t AIBU but I could really do with some help!

Filling in my passenger locator form as we are off to Spain tomorrow and I’m unsure what box to tick where it asks if you have a EU digital covid certificate or an equivalent digital covid certificate or other certificate?

I have my QR code on the NHS app and I’ve printed it out with the QR code. So is it ‘other certificate’?

I’m easily confused btw!

One more question if anyone can help.,,,

It says which vaccine did you have but on my certificate it says Astra Zeneca (Sars CoV-2) so is it AZ or Sars CoV-2 as there’s both options?

I selected AZ for hubby's. It checks the database anyway.

AZ. The sars one on there is the Chinese one.

However, we just came back from Ibiza and the authorities there just scanned the code. Didn't bother looking at anything else. Literally scan and move on.

Than' goodness, as I spelt my own surname wrong and DH picked it up the day before we flew and you can't go back and edit it. (Put a b instead of an n with fat fingers on my phone)

I've done that before. Couldn't look forward to the holiday because I was so nervous about it. It was fine though.
