It’s a person they know and they have a dialog in their head of them talking to the other person

It would depend what they mean by that.

I have conversations with people in my head all the time. For me, it's just a way of processing things. It is likely linked to my social anxiety, but on the whole I think it's just normal. Not everyone I've discussed this with does the same, but plenty say that they do.

I have no delusions that these people are real or that I've actually had a conversation with them, though. If a person did think this way, it could be something very different.
