formal complaint or pals concern?

I’m not sure where to post this I’m happy for it to be moved if there’s somewhere else I’m meant to post?
After my traumatic birth -where I had a cat 1 emcs plus other complications, I had to wait 7 months for a birth reflections appointment which contributed to my ptsd and pnd as I basically had no idea what had happened or why. So during the appointment with midwife I found out I was meant to have seen a consultant at 6 weeks pp where he would of gone over everything, checked my wound etc, this would of somewhat saved me many months of suffering mentally. The midwife said I fell through the cracks and she would write an incident report but I decided to complain to pals. They have emailed back asking if I would like to do a formal complaint or let pals deal with it & I’m wondering if anyone has any advice as to wether I should bother basically ? Some people might not think this is a big deal but it was huge to me to not understand what happened, I also complained about my awful hospital care on the ward . So basically I’m wondering What are the differences between pals and formal complaints and what will the outcomes be ?
