To ask why this clinic was allowed to reopen when the management and staff were so clearly unfitted for their task.

If you read the article it details that everything they found that was wrong was then fixed when they came back to check again. It's very concerning how bad it was in the first place but it sounds like they tried to do what they could to get up to the right standards.

It would concern me that there was other dodgy stuff going on which hasn't been noticed though.

Eve the very fact that it was so allowed to be get bad in the first place suggest that those involved were completely unsuitable people to run or work for such a business where the highest level of trust and competence is called for. These were not simply minor issues, they were systematic failures at every level.
