Do I stay or do I go?

Hello, just want another opinion. I have been very unhappy in my job for at least a year. It is a small company and it is very chaotic. I have been here for 4.5 years and have progressed as far as possible. New management came, and my new boss is an arsehole. But I do love the people I work with, I consider some of them friends for life. And the work is not bad.
I have been job hunting for 6 months as I kept swinging from wanting to quit so badly to nah I am staying. I finally got an offer from a good company with a decent salary increase. I am getting cold feet about leaving. I have had some health issues as well this year, and the current company has been quite understanding. I think they are trying to improve. New company means longer commute (45 mins), longer work hours, but clear career progression. I also like the people who interviewed me. I have asked them about emergency situations and they have said they care about their people. Whether it's true or not I dont know. So Mumsnetters, if you were me, would you stay because it's comfortable though no career progression, or do you go for the potential but you have to work harder?

Would depend on the health issues and if they were long term, likely to result in further time off being needed, and likely to be worsened by longer commute and working time and pressure.

Would also depend what I wanted from life and if I was prepared to pour more of myself into work instead of personal time.

Plus how realistic it was that I'd be capable of achieving promotion and performing in that role.

And how unhappy I was and whether I could identify things that would improve the situation.
