From a residential setting if it’s been rated inadequate? Or do they tend to leave the young people in the placement?

It's mixed - depends on the availability of other placements. If there isn't anywhere for them to go they can't move anywhere essentially. If the service has been placed in special measures and closed by CQC, often people end up in inappropriate placements or very far away.

The failings are relating to safeguarding.
