to be embarrassed about lost matb1

Long story short...

I sent my matb1 to work HR via recorded delivery. It got lost in post. I have evidence of posting.

Went to midwife and got another matb1. Went in to labour early (day after I got new matb1). Had baby. Was admitted to hospital shortly after getting home and just left yesterday. Went to take photo of new matb1 as agreed with work.

Didn't realise the hospital took my notes as I gave birth to baby. Matb1 was in with my notes...

Now need ANOTHER matb1.

Feeling embarrassed to ask my midwife, again, for another one.

Feeling very stressed. Colicky baby. Sleep deprived. Will my midwife be pissed off with me asking for another form?

I feel so stupid...

No, tell her exactly what happened. Colicky babies are really hard. She will understand.

Your midwife will give you another one no problem. Don’t be hard on yourself, these things happen. Congratulations on your baby.

I'm so crap at asking for anything when I feel like I've wasted someone's time. She moaned a bit about the last one and told me to take a photo of this one. I didn't do it. I guess in the grand scheme it doesn't matter. Argh... really needed to send it today as well as it's so late (baby was born 22nd Oct) and work still don't have it or even know I'm on maternity leave already. I hope they still pay me. I'm so stressed...

Ask her, its just a piece of paper to her. In a couple of weeks you'll never have to see her again. Congratulations on your baby!

Can the hospital give you a copy? More stress is not what you need.

@CarmelitasMango just ring hospital and say they have took it and you need it.

Although if it was me I would just ask midwife for a new one, but I've got super thick skin and couldn't give a shot if she moaned

@isthatalright I don't have thick skin at all (part and parcel of having an anxiety disorder sadly!)

They can’t issue you a new one from the hospital but should be able to copy what they have. Can you apply for a copy of your notes from them?
I have anxiety too. It was always worse when I had a new baby. Good luck.

Ring hospital get original you can apply for a duplicate there are rules at my place about when you should hand it in so i suppose it depends on policies and the leniency of your employer. Good luck
