To think two weeks is enough time to do this favour?

I am applying for a new post, and part of the application process was a personal reference form. It was two sides of an A4 page with about 4 or 5 short questions on each side. The person I asked to fill it in for me was more than happy to do it, so I gave it to them two weeks ago. They said they would fill it in as soon as possible, but I haven't received it back yet.

They have a history of being a bit flakey, but I know they're busy and have a lot to deal with at the minute. How do I approach this? I need the form back soon as it is holding up my application, but I'm reluctant to ask for it back because I feel awkward about it. Is two weeks a reasonable amount of time to give?

Tell them there is a cut off date (usually is) for applications to be completed by. I'd say "hi, just a gentle reminder that my ref needs to be in by closing date of ..... Is there anything you need off me to complete it? Thanks again for offering to be my referee" .
