So... I need advice re: breaking up the day when home with a baby (crawling baby!) all.day.long. How do you make time for yourself or do you just not?

Get out of the house. Put some 'structure' into the day, some sort of (really loose) timetable.
After breakfast I'll go to get those jobs done on the High Street (Post Office / Bank / pick up that Birthday card). Then we'll go via the park on the way home. Lunch. Nap (for baby whilst I run around getting stuff done).... etc..etc.
Being in the same 4 walls with no adult company would drive me mad.

I agree with BackforGood, you need to create a structure for your day, including getting out of the house, even if it is just a walk around the block.

When DD was a baby I used to have a general structure of:

Get up
Breakfast
Play time
Morning nap for DD/housework for me
Lunch
"Outside time" - getting out of the house, some days it was running errands, other days it was just a stroll round the park when the weather permitted, or baby groups/activity sessions
Home for DDs afternoon nap while I prepped her dinner
DDs dinner
DDs bath/bed
My dinner
Relax
Bed
