NHS Dental treatment - signing forms

I'm lucky enough to have an NHS dentist. Every appointment, I am asked to fill out the blue PR form, which says "one form must be completedfor each course of treatment".

Today, I'm on appointment 3 of the same course of treatment. I queried the need for the form and got an unsatisfactory response about it being need editing for each appointment, but that's not what the form says it's for.

Can anyone advise? Is there a scam to be had for the benefit of the practice here?

*Note there is a 3 year(!!!) waiting list for this place, so I don't want to rock the boat too hard!
