Ive just read on the printout of my medical history/medication list that an esa113 form was completed last month by a doctor in my practice i saw once for a repeat prescription. What does this form mean in relation to the job centre medical that i have next week? Im so scared that im going to fail it and lose my esa :(

It's a medical questionnaire.

You can have a look at the questions here

I found this on google

A GP will only be asked to complete an ESA113 if:

it could result in the patient’s entitlement to additional financial support being confirmed on paper evidence, without need for a face to face assessment, orif, in the case of reassessing the patient’s continuing entitlement to Employment and Support Allowance, it could result in ongoing entitlement being confirmed without need for another face to face assessment.
