He wasn't in prison, he was in a medium secure mental health unit and presumably was part of his treatment and rehabilitation. Obviously in hindsight it was inappropriate :( I hope he's detained soon

Yes - mental health unit. I think his sentence had been served.

It certainly does seem as though their procedures failed.
