It is a way of showing fake or sarcastic concern. I don't mind the odd head tilt.

You do it when you're showing concern for someone. It's just supposed to suggest the poster has taken leave of their senses, and the is concerned for their wellbeing.

I usually tend to read it as meant more humorously than nastily.

Although there was a thread a few years ago where its use kicked off a huge row.
