What do you do? Sometimes I do, sometimes I don't, sometimes slighly awkward moments are spent deciding..

Just get on with it. The awkwardness and indecision can be very irritating and will give a bad impression. On the flip side, if you always shake, you will come across as more confident and trustworthy than if you vacillate. Just decide that you will always shake hands, and then always do it. Simple.
