Aibu to think that a refund should should be received after the item has been returned? Buyer not happy with item after eBay sale - I am happy to refund when I receive it back, not before she sends it. Agree?

Buyer sends it back first otherwise they get a free item you don't have your money or your item. Agree.

If the item was not as promised, i.e. sold as new when clearly worn with stains, then seller should also refund return postage once item is received back. Agree.

Agree. Also agree you refund return P&P if not as sold.
